Auto Positive Airway Pressure (APAP)
1. Auto PAP (APAP) without a PSG with PAP initiation (PSG-CPAP) is covered for the treatment of OSA diagnosed using OCST provided the following criteria are not met.
a. Patient did not meet criteria for OCST
2. Must be ordered by a Board Certified Sleep Medicine Physician.

3. If compliance (percentage of nights with at least 4 hours of use) is less than 70% in the first 30 days of use, an attended lab PSG-CPAP titration is required.

4. If clinical symptoms are not resolved within the first 30 days of use, an attended lab PSG-CPAP titration is required.
